Php 联系人表单显示为空

我采取了一个标准的形式，我总是使用其中包括一个评论是。对于这个网站，我不需要评论，所以我只在里面留下了电子邮件和url。当我删除PHP文件中与注释相关的所有内容时，它会显示一个空白页面，甚至没有错误
